SD 455 Registry Form

This form is being used to collect 1973/1974 SD 455 VIN's.
A registry was started by Jack Janney in 1986, then Frank Gatarz.
That registry never seemed to be updated or easily accessible.

I started this registry,using some of the data that was in the earlier registry.
I will be updating this whenever new information comes in.

The personal information need not be filled in, it's just for my use to contact you if necessary.
I won't be posting the names,or email on the web.

If you would like the VIN hidden, write that in the comments section, and I can hide it.
